- Certificate of Compliance with Mandatory Disclosure (Form 12.932)
- Notice of Social Security Information (Form 12.902(j))
- Notice of Confidential Information (to accompany Notice of SSN)
- Enlarged front and back copy of driver’s license or Affidavit of Corroborating Witness (Form 12.902(i)
